Assistance with Hospital and Rehabilitation Discharge Planning
Upon discharge from the hospital, decisions may need to be made regarding a senior’s return home or to a short-term stay at a rehabilitation center. Our Care Managers can ease the stress of this process, by discussing the pros and cons of several local facilities and identifying the differences among them. Since all facilities are certainly not the same, we help the family understand what questions to ask and what to look for to ensure the right fit and best services for our client.
If an older adult is able to return to their home, we ensure continuity of care by meeting with hospital personnel and arranging for necessary supportive services such as transportation, oxygen, a caregiver, and any other assistance that is needed.
As an extension of your own family, our Care Managers continually look out for an older adult’s best interests, serving as an advocate and mediator. Our Care Managers can serve as a single source for clients’ legal documents, insurance information, and important medical documentation. When dealing with hospitals and doctors, we advocate for the patient, providing insight to doctors regarding changes with the patient’s physical or cognitive condition, listening to what medical advice is given and communicating any changes to family members and caregivers. ElderCare Solutions of Michigan serves as the line of continuity, ensuring that medical recommendations are carried out.

I wanted to tell you what a pleasure it has been working with you and the Care Managers from ElderCare Solutions of Michigan. In my many years of working with the aging population, I have seen many seniors that don’t have the support they need from family as they age and their needs change. As was the case with Mrs. F recently, who had family that very much wanted to help, they couldn’t because they lived out of state. ElderCare Solutions of Michigan was able to provide a professional Care Manager whose expertise, experience and compassion allowed the daughter to comfortably board the plane to head home, knowing her Mother was getting excellent care, direction and support… We can provide the physical care for this woman to return to her home safely, but there is a myriad of services that will need to be coordinated before she can do so.
